This packet includes new patient forms that will need to be completed in order for us to assist with your care:

• Consent for Treatment and Release of Information and Payment Authorization

• Controlled Substance (Narcotic) Agreement

  Maintained in file in the event controlled medications are prescribed by this office

• Authorization to Release Medical Records & Information

  May determine facility name at time of office visit

• Patient Information

  May omit the following sections if information not applicable or not readily available: Referral Information; Attorney, Insurance Information; and Work Comp, Personal Injury, Third Party, or Other Information
